Event Information

This Waiver and Release of Liability applies to the Participant’s involvement in the World Cup Soccer Program. World Cup Soccer is a community event operated by the Town of San Anselmo’s Recreation Department with the help of volunteers. The event is held at the Memorial Park Tennis Courts located at 1000 Sir Francis Drake Blvd., San Anselmo, CA, between June 7, 2026, and July 19, 2026.

Waiver, Assumption of Risk, and Indemnification

I, the undersigned, fully understand that my and/or my child’s, participation in World Cup Soccer Day (the “Event”) exposes me and/or my child to the risk of property damage and personal injury. I hereby acknowledge that my and/or my child’s participation is voluntary and I assume all risks.

In consideration of participating, I hereby release, discharge, and agree not to sue the Town of San Anselmo, its officers, employees, agents, and/or volunteers (“Released Parties”) for any and all claims, demands, or causes of action for any injury, loss, or damage arising out of participation in the Event, including, but not limited to, the active or passive negligence of the Released Parties, other participants in the Event, or the condition of the property, to the fullest permitted by law.

In consideration of participating in the Event, I agree to indemnify and hold harmless the Released Parties from all claims, demands, and costs, including reasonable attorneys’ fees, arising from my and/or my child’s participation in the Event.

Photo/Video Release

By signing this form, I also authorize the Town of San Anselmo to use or publish any images of me and/or my child, taken by the Town of San Anselmo at the World Cup Soccer Program for informational or marketing materials.
